**The Opportunity**

Full time, temporary for six months - Monday to Friday, 8am - 4.30pm

If you have an excellent understanding of primary and secondary services, have strong interpersonal skills, can work independently, are a critical thinker with initiative and the challenge of piloting a new role excites you then this may be the role for you.

This is an exciting opportunity for a registered nurse to be involved in piloting a new senior nursing role within MidCentral.

The Specialty Clinical Nurse (SCN) Early Supported Discharge will work across the continuum of primary and secondary services. They will provide clinical expertise and leadership to expedite early discharging and coordinator extended packages of care within the community as required. The SCN Early Supported Discharge will liaise with GP Teams, POAC (Primary options for Acute Care) Coordinator, Social Supports, ARC, Pae Ora, District Nursing, ACC and other Community Services to facilitate safe transfer of care. This will be a hospital based role working closely with our inpatient teams.
