Job Description

This is an exciting opportunity to join our team as a Case Manager. In this role, you will work closely with clients to identify their strengths and challenges, and develop tailored support plans to address their needs.

Key Responsibilities:
  1. Determine client needs through interviews and assessments.
  2. Develop and implement individualised support plans that meet client goals.
  3. Collaborate with internal stakeholders to ensure effective service delivery.
