About the Role: We are seeking a highly skilled Social Work and Healthcare Specialist to join our team at Te Whatu Ora Lakes. As a Needs Assessment and Support Coordinator, you will play a crucial role in providing holistic assessments of clients' needs and coordinating support services.
About You: To be successful in this role, you will have a strong background in social work or a related field, with experience in assessment and clinical reasoning. You will also have excellent communication skills, as well as the ability to build positive relationships with clients and their whānau.
Key Responsibilities:
- Conduct comprehensive assessments of clients' needs, identifying areas of support required
- Coordinate and facilitate access to appropriate support services, including healthcare, social services, and community resources
- Work collaboratively with clients, their whānau, and other healthcare professionals to develop personalized care plans
- Provide education and guidance to clients and their families on health and wellbeing topics
- Maintain accurate records and reporting, adhering to relevant guidelines and regulations
