Job Description

The Support Services Coordinator will play a key role in ensuring that our clients receive the support they need to achieve their goals and aspirations. This role requires strong organizational and communication skills, as well as a passion for delivering person-centered care.

Key Responsibilities

  • Coordinate the delivery of support services, working closely with clients, families, and care professionals to identify and meet individual needs.
  • Develop and maintain accurate records and reports, ensuring compliance with regulatory requirements and organizational policies.
  • Provide information and guidance to clients and their families, promoting informed decision-making and self-advocacy.
  • Contribute to the development of care plans and service agreements, ensuring that each client receives tailored support and care.

Required Skills and Qualifications

To be successful in this role, you will need:

  • A degree in a relevant field, such as human services or social work.
  • Proven experience in coordinating support services, preferably in a disability support setting.
  • Strong communication and interpersonal skills, with the ability to build relationships with clients, families, and care professionals.
  • Knowledge of relevant legislation and regulations, including those related to disability support and employment.
