The Role
We're seeking a passionate and committed Tenancy Support Co-ordinator with a particular interest in working with people who are at risk of becoming homeless and require support to maintain or start new tenancies. As a key member of our team, you'll establish effective communication and referral pathways for those individuals who are at risk of homelessness and in contact with other health and care agencies.

Your Key Responsibilities
• Establish effective communication and referral pathways for those individuals who are at risk of homelessness and in contact with other health and care agencies
• Provide wrap-around support in emergency/temporary/supported accommodations
• Prioritize caseload depending on crisis intervention and severity of needs
• Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets, and mutual aid
• Work closely with the local authority, Health & Social care sector, partners, and any provision linked to preventing homelessness and improve joint working
• Increase and enhance partnership working with primary care, mental health services, housing, and other voluntary organizations who are supporting individuals at risk of losing tenancies
• Provide wrap-around support on budgeting, education, training, and employment whilst continuing to support their engagement in structured services

About You
We're looking for someone with a minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections. Your knowledge of benefits and applying for benefits in a supportive role is essential, as well as your ability to work with and manage challenging behavior. Additionally, your experience of engaging with a wide variety of individuals with a variety of backgrounds and needs will be valuable in this role.
