About the Position
We are seeking an experienced Community Wellness Coordinator to join our team. As a key member of the Raahui Pookeka hub, you will coordinate and deliver alcohol and drug programmes, ensuring they align with the unique needs and goals of each participant.
By building strong relationships with local stakeholders and community groups, you will facilitate referrals and ensure seamless connections to additional support services. Your expertise will help drive positive outcomes for tangata whaiora and their whanau, enhancing overall well-being and quality of life.
Key Responsibilities
- Develop and maintain relationships with local stakeholders and community groups.
- Coordinate referrals to additional support services, ensuring a seamless experience for clients.
- Work collaboratively with the multidisciplinary team to achieve programme objectives.
- Maintain accurate records and reports, adhering to relevant standards and regulations.
