Role Summary
We are seeking an experienced Physiotherapist to join our ESD Team in Nelson Tasman. The successful candidate will play a key role in helping patients regain mobility and independence after hospital discharge.
This is a fantastic opportunity to work in a dynamic and supportive team environment. You will be responsible for assessing, planning, and delivering individualised treatment programs for patients transitioning from hospital to home.

About the Team
Our ESD Team is a multidisciplinary team that works together to deliver person-centred care in home and community settings. We are passionate about improving the health and wellbeing of our patients and are committed to providing high-quality healthcare services.
As a member of our team, you will work closely with other healthcare professionals to ensure coordinated care. You will also have the opportunity to develop your skills and expertise through ongoing professional development opportunities.

Your Key Responsibilities
* Assess, plan, and implement rehabilitation programs for patients transitioning from hospital to home
* Provide hands-on physiotherapy interventions to improve mobility, strength, and function
* Educate patients and whānau on safe movement techniques and recovery strategies
* Collaborate with other healthcare professionals to ensure coordinated care
* Monitor patient progress and adjust treatment plans as needed
