The Role
We are looking for a skilled Physiotherapist to join our ESD Team in Nelson Tasman. In this role, you will help patients regain mobility and independence after hospital discharge. You will work with a multidisciplinary team to deliver person-centred care in home and community settings.
As a Physiotherapist, you will assess, plan, and deliver individualised treatment programs for patients transitioning from hospital to home. You will also provide hands-on physiotherapy interventions to improve mobility, strength, and function.
Your Key Responsibilities
* Assess, plan, and implement rehabilitation programs for patients transitioning from hospital to home
* Provide hands-on physiotherapy interventions to improve mobility, strength, and function
* Educate patients and whānau on safe movement techniques and recovery strategies
* Collaborate with other healthcare professionals to ensure coordinated care
* Monitor patient progress and adjust treatment plans as needed
