About the Role
We are seeking an experienced Physiotherapist to join our Early Support Discharge (ESD) Team in Nelson Tasman. As a Physiotherapist in this role, you will play a key part in helping patients regain mobility and independence after hospital discharge.
You will work within a multidisciplinary team to deliver person-centred care in home and community settings.
Your key responsibilities will include assessing, planning, and delivering individualised treatment programs for patients transitioning from hospital to home.
You will also provide hands-on physiotherapy interventions to improve mobility, strength, and function, as well as educate patients and whānau on safe movement techniques and recovery strategies.

Key Responsibilities
* Assess, plan, and implement rehabilitation programs for patients transitioning from hospital to home
* Provide hands-on physiotherapy interventions to improve mobility, strength, and function
* Educate patients and whānau on safe movement techniques and recovery strategies
* Collaborate with other healthcare professionals to ensure coordinated care
* Monitor patient progress and adjust treatment plans as needed

Requirements
To be successful in this role, you will need to have:
* NZ registration in Physiotherapy and a current practising certificate with the Physiotherapy Board of New Zealand
* Demonstrated rehabilitation experience from either a hospital or community setting
* Strong organisational skills and the ability to work independently and as part of a team
* Excellent communication and interpersonal skills, with a compassionate approach to patient care
* Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
* Experience in projects / initiatives which give effect to Te Tiriti principals and frameworks, and the application of Mātauranga Māori and Kaupapa Māori approaches, particularly as they apply in healthcare settings
