Job Summary

\

We are seeking an experienced Clinical Assessor to join our Adult Community Therapy Service (ACTS) team. As a Clinical Assessor, you will be responsible for assessing and coordinating services for clients with complex chronic medical needs. You will work closely with our interdisciplinary team to develop coordinated care plans that support an appropriate service response.

Main Responsibilities

  • Developing coordinated care plans for Long Term Supports-Chronic Health Conditions (LTS-CHC) clients\ \
  • Completion of InterRAI assessments for adults aged 17-65 and Paediatric Needs Assessments for children aged 0-17 years\ \
  • Clear communication with clients and family/whanau to ensure their understanding of available support options and management of service expectations\ \
  • Communication with a diverse group of internal and external agencies, to facilitate appropriate support outcomes for clients and to provide advice on other appropriate resources\ \
  • Working closely with the Service Coordinator for LTS-CHC, to enable efficient workflow\
\

Requirements

To be successful in this role, you will need:

\
  • \ \
  • A minimum of 3 years' experience working within healthcare\ \
  • A high level of skill with clinical assessment and critical evaluation\ \
  • Excellent interpersonal skills, with a high standard of verbal and written communication\ \
  • The ability to work autonomously, manage time well and prioritize workload appropriately\ \
  • Registration as a health professional in New Zealand (Social work, Occupational Therapy, Physiotherapy, Registered Nurse or related discipline)\ \
  • Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth\ \
  • A full NZ drivers licence\
