**Job Overview**
This role is an exciting opportunity to make a meaningful difference in the lives of older adults in Tairawhiti. The Te Ara Tiaki Pakeke, Healthy Ageing Service has been established to work collaboratively with older adults, their whanau, health stakeholders, and the wider community to enable them to access health services and supports that promote healthy aging and respectful end-of-life care.
The successful candidate will be responsible for assessing patients for Home and Community Support Services in their homes, coordinating a team of Allied Health Assistants, and working across inpatient and community settings.

- Coordinate a team of Allied Health Assistants
- Work across inpatient and community settings
**Requirements**
- A minimum of two years of nursing experience
- Experience with coordination and scheduling
- Excellent oral and record-keeping skills
- Ability to work within a multi-disciplinary team
- Competency with Te Ao Maori, Tikanga, and Te Reo Maori or a commitment to starting your journey and taking ownership of your learning and growth (mandatory)
- Experience in projects/initiatives which give effect to Te Tiriti principals and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settings (mandatory)
