Key Responsibilities:
- Assess patients' needs and develop individualised care plans, taking into account their physical, emotional, and social needs
- Coordinate care delivery with allied health professionals and community service providers, ensuring seamless transitions and effective communication
- Maintain accurate records and reports, adhering to regulatory requirements and organisational standards
- Collaborate with multidisciplinary teams to promote best practices in patient care and contribute to quality improvement initiatives
- Develop and maintain strong relationships with patients, families, and community stakeholders, fostering trust and respect
Requirements:
- Minimum 2 years of experience in nursing, preferably in home and community care
- Strong leadership and coordination skills, with ability to manage multiple priorities and adapt to changing situations
- Excellent communication and interpersonal skills, with a focus on empathy and cultural sensitivity
- Competency in Te Reo Māori and Tikanga (Māori language and customs) or commitment to learning and growth
- Certification as a registered nurse in New Zealand, with current annual practicing certificate
