About the Job

This role requires a skilled Pathway Coordinator who can effectively manage and coordinate multiple clinical pathways and agencies for clients within regional forensic and rehabilitation services. Key responsibilities include facilitating, coordinating, managing, and overseeing these services, as well as collaborating with the Group Manager on quality improvement initiatives and reporting. Strong knowledge of national and regional resource demands for Forensic and General adult mental health services is crucial.

About the Team

We operate as part of Te Korowai Whariki, a network providing regional and national services across the Central Region. Our teams work together to deliver high-quality inpatient and community-based services that cater to diverse patient needs.

About You

We are seeking a driven, resourceful, and enthusiastic registered health professional to join our Regional Forensics and Rehabilitation inpatient service. As a valued member of our team, you will contribute to delivering exceptional care and contributing to a collaborative environment.

  • Must have excellent assessment and planning skills.
  • Previous experience in change management is required.
  • A postgraduate certificate in specialty nursing or equivalent qualification is essential.
  • Strong analytical skills and ability to apply critical thinking.
  • Superior communication skills.
