Job OpportunityThis permanent part-time role involves working as a health coach in the Access and Choice service, providing support to individuals with mental illness and addictions.
Key Accountabilities- You will work on the GP premises, in the Pathways office, or make community visits to provide support and resources to individuals.
- You will collaborate with health professionals at the practice to improve health and wellbeing outcomes.
- You must have a New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent.
- You must be eligible to work in NZ, which includes having NZ residency or a valid NZ work permit.
- A satisfactory police check is required as part of the employment process.
