The Role

This permanent full-time position requires a professional with leadership skills and experience, along with a strong understanding of the New Zealand Health System. As a Needs Assessment Coordinator Lead, you will be responsible for the operational leadership and day-to-day management of the Supportlinks service.

Key Responsibilities

  • Provide access to support services for individuals with acute/short-term needs, long-term conditions, and older people and Kaumatua">
  • Support clinical teams with discharge planning">
  • Collaborate with the community focus team">
  • Maintain accurate records and reports">
  • Develop and implement strategies to improve service delivery">

Requirements

  • ">
  • Professional health qualification with assessment as part of scope of practice (e.g., Occupational Therapist, Physiotherapist, Speech Language Therapist, Social Worker, Registered Nurse)">
  • Holds current Annual Practising Certificate">
  • Clinical experience in disability settings">
  • Functional knowledge and provision of interRAI assessment">
  • Leadership skills and experience">
  • Knowledge and understanding of the New Zealand Health System as pertains to Kaumatua/older people or people with long-term chronic health conditions">
  • Current New Zealand Drivers Licence">
