About the Role
This exciting opportunity exists within our Needs Assessment team, which is dedicated to supporting older adults in need. As a Clinical Needs Assessor, you will play a vital role in assessing clients' needs, developing personalized care plans, and connecting them with relevant resources and services.
Your Key Responsibilities
- Evaluate client needs using the interRAI assessment tool, focusing on functional abilities, medical conditions, and social determinants
- Establish effective relationships with clients, families, and caregivers to foster open communication and collaboration
- Collaborate with interdisciplinary teams to ensure cohesive care planning and delivery
- Develop comprehensive care plans that address clients' physical, emotional, and social needs
- Document all interactions and assessments accurately in digital systems
Essential Qualifications and Experience
To excel in this position, you will possess:
- A current registration as a health professional with a practising certificate
- Proven experience working with older adults in various healthcare settings
- Strong understanding of community resources and services supporting older adults
- A valid NZ driver's licence
- Proficiency in computer skills, particularly in digital record-keeping systems
