**Job Description:**

We are seeking an experienced Care Coordinator to join our team. As a Wayfinder in the Mana Ake Programme, you will be responsible for delivering early intervention health and wellbeing guidance, support, and service within Primary and Intermediate Schools.

The role involves building relationships with students, teachers, and families to provide comprehensive support and address mental health and wellbeing concerns.

You will work collaboratively with education professionals to develop and implement tailored programmes and interventions, ensuring the best possible outcomes for tamariki.

**Required Skills and Qualifications:**

  • Proven experience in working with community, tamariki, and whanau
  • Strong ability to build relationships and work effectively in a team
  • Full driver's license
  • Computer competence
