Key Responsibilities
As a Whanau Support Navigator/Social Worker, your key responsibilities will include:

  • Providing proactive support to patients and their families in the community
  • Facilitating whanau hui and planning sessions
  • Working closely with the interdisciplinary team to ensure comprehensive care
  • Communicating effectively with patients, families, and healthcare professionals

Requirements
To be successful in this role, you will need to have a minimum of 3 years' experience working in New Zealand as a registered social worker or experienced Whanau Navigator. You will also need to be a NZ Citizen or have Permanent Residency. If you are passionate about providing holistic care and working in an integrated way with all members of the team, we would love to hear from you.
