Our experienced, specialised palliative team—comprising Medical Officers, Nurse Practitioners, Nurses, HCA's, and Whanau Support staff—offers comfort, relief, and expert care when it is needed most.This role is fundamental to bringing whanau and services together.
You will be proactive and lead challenging conversations.
You will be confident to facilitate whanau hui, planning and be an integral part of the interdisciplinary team.
You will also work closely with GP teams, aged care facilities, Maori health providers, the hospital team, government agencies, home-based care providers, social work services, counsellors, and other community services.
This can be a fast-paced role that changes daily to meet patient/whanau needs.
Time management and analytical skills are essential.
