Job Summary: We are seeking an experienced mental health professional to join our Triage team as a Community Mental Health Nurse, Occupational Therapist, or Social Worker. The successful candidate will be responsible for receiving referrals, assessing clients' needs, and coordinating care with other healthcare professionals.

Key Responsibilities:

  • Receive and assess referrals from various sources, including general practitioners, hospitals, and community organizations
  • Determine the level of need and urgency of each referral, prioritizing cases accordingly
  • Coordinate care with other healthcare professionals, including nurses, doctors, and social workers
  • Provide education and support to clients and their families on mental health issues and available resources
  • Maintain accurate records and reports, adhering to confidentiality and data protection policies

Requirements:

  • Current annual practicing certificate enabling working in the mental health sector
  • Experience working within the mental health field, preferably in a triage or assessment role
  • Strong interpersonal skills, therapeutic engagement, critical thinking, and communication skills
  • Able to work effectively in a multidisciplinary team and autonomously
  • Good understanding of the community and services available
  • Knowledge and understanding of Te Tiriti o Waitangi
  • A current full driver's license
