The Adult Community Therapy Service prioritises the care and wellbeing of clients/whānau. We work collaboratively with clients, their families, and healthcare professionals to develop effective care plans that address their unique needs.

This role involves coordinating services to support clients in living at home, as well as facilitating transitions to supporting living or care facility options. You will work closely with our experienced interdisciplinary team to deliver high-quality, patient-centred care.
