Disability Support Coordinator

As a Disability Support Coordinator, you will work closely with families to identify their goals and develop tailored support plans. Your tasks will encompass a range of activities, including grocery shopping, meal planning, preparation and cooking, laundry, light cleaning, and household support.
We are seeking a skilled and empathetic individual who can provide simple, flexible supports that enhance the life of each family. The ideal candidate will possess excellent communication skills, be respectful of others' thinking, and demonstrate cultural awareness.
Key responsibilities include:
- Creating support plans that meet the unique needs of each family
- Providing assistance with daily living activities
- Collaborating with families to identify their goals
- Tailoring supports to meet the changing needs of each family
