Role Summary
The Home Health Licensed Practical Nurse (LPN) is responsible for providing personal care and routine skilled nursing services to patients in their homes under the direction of a Registered Nurse. This position requires strong communication skills, attention to detail, and the ability to work collaboratively as part of a team.

Responsibilities
* Use and prepare equipment and supplies effectively and efficiently adhering to aseptic technique
* Implement plan of care initiated by registered nurse
* Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
* Provide accurate and timely documentation consistent with the plan of care
* Participation in Case Conferences
* Provide direct patient care as defined in the State Nurse Practice Act

Qualifications
* LPN Program Graduate
* Valid DL with no more than 3 moving violations
* Proof of current/valid car insurance
* Knowledge and experience with electronic health records
* BLS Certification

Benefits
