Interventions are tailored to individuals and may include medications as well as information about osteoporosis and lifestyle measures that can assist bone health and reduce risk of future fractures.
The service is now district-wide operating across the Hutt Valley and Capital Coast areas seeing people in our inpatient wards and in outpatient appointments.This is a fixed term .65 fte role as our current CNS is about to take parental leave.
This role is a non-prescribing role and a great opportunity for someone who has a special interest in bone health and a strong commitment to the health and well-being of Older People.You will be assisting with identification of people for further assessment, completing bone health and falls risk assessments, completing follow-up tasks, ordering investigations, formulating long term treatment plans for bone health, offering preventative education and, where indicated, referring to our prescribing clinicians for initiation of treatment.
We also maintain relationships with Primary Health and Community based falls prevention services.
The person in this role will be comfortable working in inpatient wards, gathering information from patients over the phone, and making recommendations to support inpatient teams in their treatment of bone health as this is a key function of the CNS role.The Fracture Liaison team is small and friendly with a Pharmacist/Team Coordinator, a part time Nurse Practitioner, two Administrators and a 0.2 FTE Geriatrician Clinical Leader along with this CNS role.
We interface with Emergency Departments, Orthopaedics, Geriatrics, Endocrinology, Rheumatology and Radiology within the hospital, also taking fragility fracture referrals from Primary Health.
The Coordinator in this role will receive a solid orientation and support from existing staff whilst the team as a whole can access day to day collegiality from the large District-wide Community Older Adults service.You will be a CNS practising at an advanced level with in-depth knowledge and experience of bone health.
You will help establish and embed a best practice FLS, achieving International Osteoporosis Foundation (IOF) accreditation.
As this is a Coordinator role we are looking for someone with great interpersonal and networking skills, who will be a supportive team player.
The primary location for this role will be Wellington Hospital and Kenepuru Hospital.
Team meetings and education will be based at Hutt Hospital.
You will often be working away from other team members, requiring a degree of clinical autonomy.
