In this pivotal role, you'll lead with 24/7 accountability to ensure the highest standards of clinical excellence in home health patient care.
Leveraging evidence-based practices, cutting-edge management theory, and exceptional organizational skills, you will drive our mission to deliver outstanding patient outcomes.
Your visionary administrative leadership will craft an integrated patient care system, fostering innovative and collaborative management models.
Working closely with a dedicated team of healthcare professionals, you will build a culture of continuous improvement and teamwork, ensuring that our patients receive the best possible care.This position requires a minimum of one year experience in a supervisory capacity in a Home Health agency.ResponsibilitiesUtilizes the quality process and best practices to improve clinical operation of the home health agency.Utilizes guidelines, standards and research to support an interdisciplinary care delivery system that produces quality patient care with optimal patient outcomes.
Incorporates data and information to continually improve operations.Ensures quality patient care by directing the management of an interdisciplinary patient care delivery model that supports optimal patient outcomes.Ensures institutional compliance with professional, regulatory, and government standards of care.Monitors customer satisfaction and seeks ways to continually improve customer satisfaction/service.Participates in and supports quality/performance improvement initiatives and demonstrates sound project management skills.Demonstrates effective communication skills by communicating in a direct, open, honest manner.Provides staff with timely feedback.Effectively disseminates information about organizational mission, vision and strategic imperatives and the expectations of the staff in relation to the strategy.Provides support and guidance to enhance team development by assisting team with the identification and achievement of professional growth opportunities.Utilizes the performance appraisal in a fair manner and assists team with developing career goals.Acts as a role model for team, mentors and coaches team and peers to achieve professional career goals.Creates a supportive environment that allows team to take risks to further develop.Engages team to be involved with the agency's team engagement initiatives.Makes self-visible to team by providing opportunities to meet with team, routine home visits with team, and available office hours.Handles conflict and resolution of conflict in a timely and appropriate manner.Provides timely follow up with team: treats team in a fair, consistent manner yet recognizes individual differences in the team.Monitors team satisfaction quarterly and implements initiatives to continually improve team satisfaction.Provides routine recognition of team, both formal and informal: supports team at recognition events.Maintains own clinical knowledge base and ensures staff maintain competencies outlined in the performance appraisal.Creates and maintains respectful relationships throughout the Organization and external customers.Encourages cross-boundary involvement in obtaining agency goals.Works with peers and others in operating unit and throughout the system to achieve goals.
