About the Role
In this challenging yet rewarding role, you will have the opportunity to work closely with patients and their families to provide direct care services, act as the primary liaison between the physician, patient/family, and hospice team, and maintain accurate clinical records. Your primary responsibility will be to report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary control and professional management of each patient.

Main Responsibilities:

  • Provide direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load.
  • Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff.
  • Maintain all assigned patient clinical records.

Requirements:
To be successful in this role, you must possess a hospice heart – being compassionate, kind, and empathetic towards patients and their families. Additionally, you should be comfortable providing care wherever the patient calls 'home', demonstrate flexibility and composure while supporting others in times of crisis, and have a valid RN license in the state of requested employment (or eligibility to obtain).
