About the role The Respiratory team at Starship Hospital is seeking a permanent, part-time (0.8FTE) Nurse Specialist and Co-ordinator for the Starship Long Term Ventilation Programme.
This position sits within the Respiratory Service but will also be closely involved with the Allied Health and Paediatric ICU teams.
As a Nurse Specialist in this role, you will support children, young people and their whānau (families) requiring life support home ventilation.
You will: * Coordinate formal pre-assessment processes, and support whānau entering the pathway throughout their healthcare journey to facilitate shared decision making * Organise the discharge process including MDT involvement; education for whānau, overseeing of equipment and liaising with the local Health NZ Districts to formulate a care package.
* Work both collaboratively and autonomously to co-ordinate the MDT and provide specialist advice with a strong patient and whānau focus.
* Provide case management following discharge from hospital, including outreach to local Health NZ Districts to children and their whānau, and support of health professionals involved in any on-going care * Coordinate multi-disciplinary and multiagency teams to ensure each child can live their maximum potential and in the best environment.

About the Service Starship Child Health is a dedicated paediatric healthcare service and major teaching centre, providing family centred care to neonates, children and young people throughout New Zealand and the South Pacific.
Starship is a part of Health New Zealand | Te Whatu Ora and is the only comprehensive children's hospital in New Zealand serving a population of approximately 1 million children.
Auckland City Hospital is NZ's largest public tertiary hospital which has strong ties with University of Auckland and provides extensive teaching and training opportunities.
About you To be successful, you must uphold strong leadership skills and have proven advanced knowledge of the management and support of children and young people with complex medical conditions.
