These roles directly deliver the services of various Branch programs to clients in communities across Ontario.
They are responsible for maintaining standards of client service delivery that align with established processes and guidelines, and are responsible for documenting accurate and timely case notes.
They are required to make independent decisions that reflect good judgement, maturity and ethical behaviour while maintaining appropriate boundaries.
What you will be doing: Delivery of associated program goals including delivery of services, managing volunteers, maintaining and supporting clients within a pre-defined geographic areaProvide supports to assist clients in developing skills to deal with and resolve identified challengesMaintain client files including case notes for pertinent data collection, ensuring client confidentiality at all timesPerform administrative duties related to case management and service deliveryEnsure program delivery is consistent with funding agreements, including community engagement and system navigation supportPlan and execute on a client service plan, including referral to other agencies that provide financial assistance, legal aid, housing, medical treatment and other servicesEvaluate the effectiveness of service and program deliveryOrganize and facilitate community events to enhance client and community knowledge, improve client access, and provide cultural sensitivity trainingMake home visits to assess client's needs and provide supportsOther reasonable duties as may be assignedQualifications: Completion of post-secondary education in a related field, such as Social Service Worker, or equivalent combination of education and experienceClient service experienceDemonstrated social services/community supports experienceHigh degree of technology literacy and proficiency in standard office equipmentAdvanced communications skillsOrganization skills and time/priority-managementHighly detail-orientedDemonstrated ability to problem-solve, demonstrate good judgement, and make decisions independently within set processes and proceduresDemonstrated ability to work alone/independently with minimal supervisionPreferred Qualifications: Knowledge of indigenous (preferably Métis) culture, issues, and current activitiesOne year or more experience in a similar/comparable roleExperience dealing with confidential informationExperience working in coordinating and delivering community-based programs or projectsAODA Statement:
