Background Community Link Upper Clutha, based in Wanaka, operates as a registered charity under the Alpine Community Development Trust.
Our vision is to create a community where everyone in the Upper Clutha can thrive.
Our purpose is to serve as a one-stop hub for community support and connections, helping to link people with the information and services they need.
As the public-facing centre for community support, Community Link plays a vital role in connecting people to information and assistance.
In Upper Clutha, the demand for these services continues to grow.
Description Working closely with the team at Community Link, the Social Worker/Health Professional will take a lead in delivering appropriate and timely intervention to clients.
Key Responsibilities It is envisioned that ideally the Social Worker/Health Professional role will be delivered over 4 - 5 days to support the function of Community Link.
Responsibilities include: Assessing, planning, and reviewing appropriate interventions with CL clients Developing systems and accountability processes to support this service Maintaining strong working relationships with other professionals in the sector Establishing referral pathways to other providers Gathering feedback regarding changing and evolving needs in the community Identifying gaps in service delivery and liaising with the CL team regarding creative solutions Other tasks, such as report writing, as required Desired attributes and skills Relevant qualification including understanding of cross-sectoral issues that can impact on an individual/family whanau wellbeing Sound engagement and assessment skills necessary for triage work Understanding of bicultural practice and responsibility as a Treaty of Waitangi partner Strong organisational skills and ability to work independently Flexibility to meet evolving and changing needs of the job Good working knowledge of Microsoft Office suite and/or other technology Knowledge of the community and the resources available Knowledge of government and non-government services Hold a current NZ driver's licence Police vet check required Key Relationships Reporting to the Community Link Manager, working closely with the Community Link team in a supportive, close-knit environment.
