Senior Care Coordinator - Wellington

We have a full-time position available for a Senior Care Coordinator to join our supportive and friendly team based in our Petone office.

To coordinate home-based care and support services that enable Access clients to live as safely and as independently as possible in their homes and communities.

**Nga mohiohio e pa ana ki tenei mahi | Key Responsibilities**
- ** Coordinate Home-Based Care**: Oversee care and support services to ensure clients live safely and independently.
- ** Build Relationships**:Develop strong connections with team members, clients, and whanau (family) to foster trust and communication.
- ** Deliver High-Quality Service**:Ensure care plans and services are delivered to the highest standards, in line with policies and procedures.
- ** Manage Support Workers**:Coordinate placements to ensure optimal care and client satisfaction.
- ** Adapt to Client Needs**:Respond flexibly to the unique and changing needs of clients within a structured framework.
- ** Complaint Resolution**:Lead the team in addressing and resolving client complaints, escalating issues when necessary, and finding effective solutions.

- Experience in team management, staff training, and rostering.
- Strong interpersonal skills with the ability to build relationships across diverse groups and resolve conflicts.
- Proficient in Microsoft Office and general computer skills.
- Excellent organisational and time management skills with a keen eye for detail and accuracy.
- Ability to work under pressure, multitask, and adapt to changing priorities.
- Commitment to upholding Te Tiriti o Waitangi principles in the workplace.
- Eligibility to work in New Zealand.
