**Hours: Permanent, full-time (40 hours per week)**

**Job specific information**:
This role is to provide family support, advocacy, liaison with other agency and co-ordinate services within the Early Intervention Team, CCS Disability Action Christchurch. The position is responsible for working in partnership with family and whānau of the children referred, the Early Intervention Team and any other individuals or agencies to support, develop, deliver and/or coordinate a range of family-directed services that aim to empower and enable the whānau to achieve their goals and for their children to reach their full potential.

**About the role**:

- Coordinate high quality services that are creative, flexible, and responsive to the needs of disabled children and their families / whānau. Services focus on improving / maintaining inclusion of disabled children within their families / whānau and the wider community.
- Coordinate delivery of contracted services which meet the funders’ specifications.
- Work in ways which are consistent with the te Tiriti o Waitangi.
- Maintain a high standard of professional and organisational behaviour and practice.
- Recognise safety issues for children, young people and vulnerable adults, respond to these and refer to appropriate supports / organisations (internal and external).
- Attend, participate in and seek training / professional development opportunities (including internal and external supervision).
- Develop and maintain community connections and networks; and promote the concept of inclusion and community development.
- Develop and maintain positive working relationships with disability consumer groups and governance.
- Undertake general administration and other tasks as agreed with your Service Manager and as required to support the team.

**About you**:

- Relevant Degree or Diploma in Social Services or equivalent qualification.
- New Zealand Registered Social Worker.
- Experience working in a social services agency, and in particular working with disabled children / youth / adults, their families / whānau and significant others.
- Demonstrates a willingness to be creative and innovative in delivering services.
- Networking skills and knowledge of government, non-government and private sector services available within the community.
- Understanding of person and family-directed planning and community development concepts.
- Is committed to providing quality customer services.
- Knowledge of relevant legislation and government policies.
- Full NZ driver license.
- Lived experience of a having a disability would be advantageous.
- Strong time management and organisational skills.
- Excellent written and verbal communication and reporting skills.
- Understanding of and commitment to Te Tiriti o Waitangi.
- Some flexibility with hours of work may be required to accommodate individual needs.
