Kōrero mō te tūranga - About the role
Our Early Supported Discharge team works to prevent avoidable admissions and to support timely transfer home for ongoing health needs assessment and recovery planning. One of our Nurses will be taking parental leave so there is an opportunity for someone interested in this area to work up to 32 hours fixed term.
Our current RNs work regular days across the fortnight so you can too. We are looking for someone who will complement our nursing team by working alternate Saturdays. Taking into account the needs of the service, there is some flexibility as to how the remaining hours are worked and if you wish to try a community role, but can - t work 32 hours there is flexibility for fewer.
Many of our patients are older adults so you definitely will have a special interest in caring for this population group. The role includes completing comprehensive assessments and referring to colleagues/other services depending upon the needs of your patient. We are now seeing people discharged home for rehabilitation following an injury, so you will work closely with Allied Health colleagues, bringing a restorative/rehabilitative approach to your management and rehab plans.
Mō tō mātou rōpū - About our team
The ESD team has a range of professionals including Nursing, Speech Language Therapy, Occupational Therapy, and outstanding Rehabilitation Assistants as well as access to specialist Doctors. Whilst you will have plenty of opportunity to use your nursing skills, you will also be involved in our inter-professional practice pilot acquiring new talents from your allied health colleagues and sharing yours with them. The team is currently undertaking training in Te Tiriti with a view to understanding how we can implement Te Tiriti centred practice both with our whānau/patients and in how we support each other. A commitment to this learning is essential for success in the role.
