**Service/Department**: The PARIS team

**Contract**: 80 hours per fortnight (1.0FTE) _open to negotiation_

**About the role**:
The PARIS team is looking an enthusiastic, innovative, motivated and dynamic health professional to join our team and continue supporting the smooth running and co-ordination of patient flow within our hospital.

You will be responsible for assisting the Team with early identification, individual management plans, strategies to increase patient safety, supporting safe and effective discharges, developing new policies, education and training to staff. You also will need to be proactive in identifying concerns or issues that may delay effective discharges and work with the Ward Teams to overcome and limit these.

**Skills and experience**:
This is an excellent opportunity for a Registered health professional with demonstratable experience managing complex patients. Excellent communication and organisation skills are essential; and flexibility as the role involves day-to-day liaison with multi professional colleagues, patients and relatives. Thorough knowledge of the hospital values, policies, pathways and procedures is essential for this role.
