**Paulownia Trust Community Support Worker Purpose**: To provide support to people with disabilities, that assists them to have great lives at home, in the community and in their leisure time.

Paulownia Trust provides support to individuals who primarily have intellectual disabilities. At times community support workers will work alongside the people we support individually, and at other times will provide support in partnership with their other colleagues.

Paulownia Trust staff are guided by the dreams and aspirations of the people we support. The role of the community support worker includes supporting people to realise their dreams and aspirations to the extent that they cannot do this independently.

**Reporting to**:

- The shift team leader and ultimately the manager

**Functional Relationships with**:

- The residents
- Residents’ families and friends
- Colleagues
- Community support services such as Horizons Day Programme and Friendship Club
- Community professionals such as Therapy Professionals and Nurse Maude

Key Resident Responsibilities:

- Provide person-centered support that enhances the individual's health and wellbeing.
- Respect the voice and rights of all individuals supported by Paulownia.
- Support individuals to make their own decisions and choices.
- Actively support individuals in day-to-day life, including their personal cares.
- Help each person to develop and maintain family relationships and community friendships and connections
- Transport individuals to their appointments, day programmes and events along with assisting those who wish to use public transport
- Administer medications as per the Medication policy.

**Key Organisational Responsibilities**:

- Communicate effectively and professionally.
- Follow Paulownia Trust policies and procedures, including Health and Safety.
- Inform your Manager, Team Leader, or Health & Safety Officer of any Health and Safety concerns
- Comply with the Paulownia code of conduct.
- Complete administration tasks (daily notes, time sheets, incident reporting etc) in a professional and timely manner.
- Assist with all household duties as per duties list.
- Be a positive contributing team member and attend staff meetings where possible.
- Always represent the organisation in a professional manner
- Undertake other tasks as directed by the team leader or manager.
- Seek support and guidance from the team leader when situations are new or difficult to resolve.

**Person Specification Skills**:

- Reasonable computer literacy
- Must have full, clean drivers' licence.
- Good communication and people skills
- Good problem-solving skills
- Good cooking and housekeeping skills
- Fully vaccinated.

- Have a good knowledge of your community - events, groups, and opportunities.
- Adaptable, able to be flexible and make changes when needed.

**Job Type**: Casual

**Salary**: $22.70 - $28.25 per hour
