**Main responsibilities**:

- Assist clients in addressing the needs identified in planning reports
- Ensure successful rehabilitation outcomes - addressing emergency needs
- Provide brief social work intervention when needed e.g. counselling when needed
- Ensure clients and their whānau are kept well informed of our support services and resources

- Degree in social work or equivalent
- Annual Practicing certificate as registered Social Worker.
- Membership of SWRB and ANZASW
- Previous experience with Gensolve would be advantageous
- Multidisciplinary team collaboration Skills
- Proven track record of achieving KPIs is desirable
- Experience with ACC contracts and stakeholder management is desirable
