An exciting new opportunity to create new roles have become available for highly motivated registered nurses, who would like to be part of a Pilot Initiative, over next 4 months. Ideal if you are needing a change while looking for the next thing - this work will have a focus in Planned Care contributing towards achieving equitable outcomes for Pacific. Should this pilot be successful, we will be proposing continuation t for the further development of the roles. Situated within Fanau Ola - Pacific Health, you will play a key role in working under the lead of the Clinical Nurse Specialist - Pacific (Planned Care) and other Pacific Health Navigators to implement strategies to ensure care pathways mitigate avoidable, unjust or unnecessary barriers caused by the system of health care for Pacific.

The Pilot aims for the Planned Care Pacific team to work closely with our planned care services / outpatient sites to improve the patient journey and access to timely care, particularly for Pacific patients in CM Health. You will participate and fulfil strategies that best support Pacific to stay on their planned care pathway journey, advocate and build effective relationships and networks to ensure culturally responsive care and treatment are accessed. You will work within the Fanau Ola, Planned Care Pacific Team, in a supportive environment who are dedicated to achieving Equity in healthcare for Pacific peoples.

This position will contribute to Counties Manukau Health (CMH) achieving health equity for Pacific by supporting Pacific workforce to develop their capacity and capability to respond to the needs of Pacific peoples and communities.

**Essential**:

- Qualified Registered Nurse
- Current NZ Practicing Certificate
- Experience and understanding of cultural knowledge working with Pacific patients and communities
- Experience working with complex health and social needs
- Passion to address Equity for Pacific Health
- Preference for able to speak second language fluent.
- Excellent written and verbal communication skills
