Purpose of the Role, Whāinga te āhuatanga: To provide psychosocial assessment, support, and information to people with cancer and their whānau. This includes support and/or care coordination needs related to primary or recurrent cancer and those receiving cancer treatment.

Social Work Service Objectives:

  • Contribute to the continued growth and development of the social work service in keeping with socio-political changes in Aotearoa.
  • Positively impact on inequitable cancer outcomes for Māori.
  • Assist patients and their whānau with complex social needs to access the help they need.

Key Responsibilities, Tasks, and Performance Indicators:

  1. Contribute to enhancing the service:
  • Collaborate with the Northland Social Worker, Northland Nurse Team Leader, and Auckland Nursing Services Manager to review and adapt work streams and service parameters.
  • Develop measures to evaluate impact in partnership with the Northland team.
  • Coordinate with other Supportive Care services and community agencies.
Positively impact on inequitable cancer outcomes for Māori:
  • Embed support for Māori patients in access criteria and support broader initiatives like Kia Ora E Te iwi and cancer prevention efforts.
Assist patients with complex social needs:
  • Conduct psychosocial assessments and develop care plans using the Te Whare tapa whā model.
  • Communicate effectively with relevant professionals regarding patient risks and needs.
  • Provide counselling on social and financial impacts, facilitating access to external resources.

Experience and Qualifications:

  • Degree in Social Work, current APC, Registered with SW Registration Board, driver's license.
  • 2 years practical experience in social work.
  • Experience with clients with physical health conditions and families/domestic violence.
  • Knowledge of health and disability sectors, social work counselling skills, IT proficiency, and psychosocial assessment competencies.

  • Understanding of addressing health inequities and working with diverse populations.
  • Excellent communication skills, empathy, and biculturalism knowledge.
  • Ability to prioritize workloads, adhere to professional ethics, and document interactions accurately.
  • Networking abilities, team orientation, and efficient time management.

Key Relationships:

  • Manager Supportive Care, Nursing Services Manager and Nurse Team Leader, Northland Social Worker, Auckland supportive care services teams, external cancer support agencies, government agencies, and community-based organizations.
