Our team in Palmerston North is looking for a Health Navigator to support the roll out of the new Integrated Care Pathways in the region.

We will provide you with the growth and tools you need to succeed in the role and look after people the way they deserve to be taken care of - the Habit Health way.


Main responsibilities:

  • Liaise with the client and physiotherapy/rehabilitation team to ensure rehab is progressing towards client outcomes, and any additional concerns are managed in an appropriate and efficient manner
  • Development and management of local relationships with clinical partners/providers including but limited to Specialist's, GP's, Primary Health Care providers
  • Ensure any cultural needs are considered and addressed appropriately and onboard clients onto the Behavioural Change Platform and explain how this fits into their care and ECP experience/journey
  • Coordinating providers within the service to execute the management plan to ensure the greatest efficiency of the treatment
  • Regular communication with client and clinical team to review progression toward client outcomes

About you:

  • Excellent ACC operational, medical and policy knowledge associated with rehabilitation industry
  • Microsoft Office 365 (highly proficient)
  • Bachelor of Physiotherapy, Occupational Therapy, Social Work or Psychology (desirable but not essential)
  • Annual Practicing certificate as registered health professional (if appropriate)
  • 3 years + rehabilitation experience
