About the Organisation:

This organisation started as a small GP clinic and has grown to be the Largest kaupapa Māori healthcare organisation in the Western Bay of Plenty, including two GP Clinics with supporting services Pēpi Ora/Well Child, Kaupapa Māori Nurse Services, Oral Health Programs, Mental Health & Whānau Centred Services.

Te Manu Toroa aims to contribute to the health of their community by being responsive to their holistic health needs, through a kaupapa Māori framework. By achieving a state of total and complete health, they know their clients can be confident in themselves to live life to the fullest and to engage in the wider world

Te Manu Toroa Trust is seeking a dedicated experienced Health coach to join our team. As a vital member of our Clinics department, the successful candidate will play a crucial role in delivering high-quality healthcare services to our community. This position offers an opportunity to make a meaningful impact on the well-being of others.

Responsibilities:

  • Strong desire to help our clients achieve their Health Goals
  • Providing comprehensive care to patients within the general practice setting
  • Supporting, our clients with planning, and implementing care plans in collaboration with the General practice team
  • Supporting education and wellbeing as prescribed by healthcare clinicians
  • Supporting patients in managing chronic conditions and promoting preventive health measures
  • Maintaining accurate and up-to-date records and documentation
  • Participating in quality improvement initiatives to enhance patient outcomes and satisfaction

About You:

  • Hold a certification in Health and Wellbeing or Personal Trainer qualification
  • Hold or be willing to complete first aid certificate
  • Prior experience as a Health Coach in a primary care or general practice setting is highly desirable
  • Strong communication and interpersonal skills for effective patient engagement
  • Ability to work autonomously as well as collaboratively within a multidisciplinary team environment
  • Commitment to Māori Health and wellbeing
  • Knowledge and experience with working in a tikanga Māori framework/ indigenous Māori nursing models of health.
