Āheinga | The Opportunity

We are seeking a Health Improvement Practitioner to work within general practice clinics as part of an integrated care team. You'll deliver 15-30 minute brief interventions, supporting individuals, whānau, and groups to identify and achieve meaningful health goals. You'll collaborate closely with GPs, nurses, Health Coaches, and community providers to ensure patients receive the right support at the right time.

Your work will make a lasting impact—improving access to mental health and wellbeing support within primary care and empowering people to live healthier, more connected lives.

Ōu Pūkenga | What You'll Bring

  • A relevant qualification regulated under the Health Practitioners Competency Assurance Act e.g. psychology, social work, counselling, nursing, or occupational therapy).
  • A current Annual Practising Certificate (APC).
  • Experience using evidence-based therapeutic models such as Cognitive Behavioural Therapy (CBT), Acceptance and Commitment Therapy (ACT), or Solution-Focused Therapy.
  • Excellent communication and relationship-building skills, with the ability to work effectively in a multidisciplinary team.
  • A strong understanding of culturally safe practice and a genuine commitment to improving outcomes for Māori and Pacific communities.
