**Mō te tūnga - About the Role**
The Canterbury Hauora Co-ordination Hub was originally established to support people with the COVID 19 Omicron variant to safely isolate. The Hub continues to do this work but also provides a range of other services to support both primary and secondary health services. These include work to avoid unnecessary admissions, support earlier discharge and assist primary and community agencies to keep people with health and / or social complexities, well in their own homes. The Hub - s - 5 C - s - model of care is working with Complexity, using the skills of Communication and Co-ordination with the goal of Connecting people to supports and resources - always with a lens to identify and find ways to meet Cultural support needs. Much of our work is with people who are not currently enrolled in General Practice. We are also further developing a - Long COVID - project to co-ordinate the work and support those impacted by delayed recovery from this illness.
We are co-located with a small team from the Ministry of Social Development and work collaboratively (with client consent) to bridge the gaps where both agencies are involved.

- Awhina and manaaki our tangata whaiora and whānau
- Connect and engage with tangata whaiora to understand their cultural requirements
- Work alongside and guide our health professional colleagues to be culturally responsive
- Tautoko (support) our kaimahi (staff) in understanding Tikanga Māori

- A natural ability to build and nurture relationships
- Previous experience working in partnership with mana whenua and iwi
- A reflective communication practices and a holistic view when caring for others
