The Bay of Plenty Care Coordination Centre provides a single point of access, navigation and coordination to people requiring short term health services across the Bay of Plenty.
We are currently seeking a Community Care Coordinator (non-registered) for a permanent position based at Te Whare Whakamana office located at the Business Centre 17th Avenue, Tauranga.
This position requires adeptness to connect compassionately and collaboratively with clients and health care professionals, via telephone and technology-based communications. The service goal is to support clients access the best healthcare services for their needs.
As a Coordinator you will be responsible for screening requests for assistance, identifying the most appropriate approach and initiating this response. You will be building relationships with key health care providers in the community and the hospital.
He Korero mōu - About You
To be considered for this position we require the following skills and experience:

- Significant experience working across the Health and Disability sector
- Excellent working knowledge of key health partners and community services
- Experience in triage, assessment and implementation of an enablement approach

All Care Coordinators are required to be digitally competent. You will be required to collaborate well with others and have a solution focused attitude.
