An exciting opportunity has arisen for an innovative professional to work full or part-time as part of the wellbeing team within Stewart Street Surgery and Te Kotuku Hauora, to enhance the ability of the general practice team to provide holistic care to patients.
We are looking for a clinician or allied health professional with an interest in mental health, to provide evidence-based brief interventions to individuals, whānau and groups. The Health Improvement Practitioner is based within general practice and works closely with the general practice team, Health Coach, Iwi organisations and other NGOs and community partners, to increase access to brief interventions to improve patients' mental and physical health, supporting their holistic wellbeing. You will also be a key part of the general practice team providing feedback, education, advice and practice pathways, to put mental health and wellbeing at the heart of general practice to support population health outcomes.
The role is part of a new model of integrated primary mental health and addictions support, which aims to provide all New Zealanders experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centered care and support. As a result, this role will best suit someone comfortable working with change, who is excited by the opportunity to improve and refine the model of care to best meet the wellbeing needs of the community.
We are looking for someone who;
- Holds current New Zealand registration as a health professional in Nursing, Psychology, Occupational Therapy, Counselling or Social Work.
- Has experience building therapeutic rapport with clients and ideally providing evidence-based talk therapies such as FACT, ACT and/or CBT.
- Has highly developed interpersonal skills and the ability to build relationships with within Māori, Pasifika, Youth and wider community networks.
- Has excellent computer skills and ideally experience recording notes in electronic patient records.
- Holds a current full NZ drivers' license.
- Is passionate about learning and refining a new model to best meet the wellbeing needs of the population.
Successful applicants will receive comprehensive training and ongoing mentoring, which will appeal to clinicians with an interest and expertise in behavioral health. National training for this role will be provided by Te Pou, with dates to be determined during recruitment.
