**About the role**

Our Social Work team is part of a collaborative health system, where everyone works together to achieve positive health outcomes for patients, their families/whānau and communities. This role provides Social Work Services to designated areas of work within Older Persons Health and Rehabilitation in-patient settings.

In this role, you will be responsible for:

- Working in accordance with Social work protocols developed for the service in relation to inpatient assessment & rehabilitation, and associated community areas of work
- Social work bio/psych/social assessments, interventions, discharge and rehabilitation planning in accordance with professional social work standards and service requirements for cases referred
- Respond to health and disability needs that impact on the wellbeing of individuals and their family/whānau
- Undertake a range of statutory functions and responsibilities relevant to legislative requirements, this may include: child protection, mental health services and vulnerable adults
- Providing a consultation service to the service and community
- Action social work interventions, requests for information, and providing reports within adequate timeframes

**About You**

We’re looking for a social worker to join our inter-disciplinary team full time on a permanent basis, however part time may be considered for the right applicant.

Your skills will include:

- Flexible and adaptable approach to your work
- Desire to work in a clinical setting
- Proven experience as a strong team player
- A real interest in the health and well being of older adults and the area of rehabilitation
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
- Strong interpersonal skills, with ability to form relationships with clients and whānau under stress
- Registration under the Social Workers Registration Act 2003 with current APC
