We provide health and disability services to more than half a million people living in central Auckland, regional services for Northland and greater Auckland, and specialist national services for the whole of New Zealand. **Te whiwhinga mahi | The Opportunity**

We have a full-time permanent position (40 hours/week) available for a highly motivated **Nurse Practitioner**. Join the Intermediate Care team, Part of the Adult Community and Long-Term Conditions Directorate, in this role the Nurse Practitioner/Senior Nurse will:

- Practise independently and in collaboration with other professionals to promote health, prevent disease and to diagnose, assess and manage health needs.
- Provide assessments and treatments interventions, including differential diagnoses, ordering, assisting with and interpreting diagnostic and laboratory tests, and administering therapies for the management of actual or potential health needs.
- Work in partnership with individuals, whānau and communities across a wide range of settings.
- Provide nursing leadership, quality initiatives and contribute to service and directorate development

Our team compromises of:

- Rapid Community Access Team (RCAT), an intensive, short term service that provides acute Nursing and Allied Health care for adult patients,
- Interim Care Scheme (ICS) providing access to beds in Age Related Residential Care facilities (ARRC) to support patients who don’t need to be in hospital, but are not yet ready to be supported in their own home
- Hospital in the Home (HiTH) that delivers hospital-level acute health services in the patient’s home.

Supporting whānau choice and timely hospital discharge while delivering ‘Person and whānau-centred care which empowers everyone to manage their own health and wellbeing, giving people, their carer’s and whānau meaningful control’.

Currently implementing additional patient pathways including heart failure, COPD and cellulitis. HiTH/RCAT and ICS provide a safe alternative option to hospital care.

**Nga Pūkenga Motuhake | Important Skills**

We are seeking a nurse with:

- Registration with Nursing Council of New Zealand (NCNZ)
- A current Annual Practicing Certificate (APC) or in the late stages of obtaining your registration, if overseas
- Current registration as a Nurse Practitioner with a minimum of 2 years of meeting nursing council requirements for a Nurse Practitioner.
