Our Access and Choice Community Support Workers partner with other health and wellbeing practitioners based in GP practices such as Doctors, Health Improvement Practitioners, Health Coaches and Social Workers.
Our Tangata Whaiora / clients we serve, will have a range of challenges from mental health difficulties, wellbeing problems or day to day barriers that have grown to be just too much to manage.
A core tenet behind this is 'walking alongside someone on their journey' and this is the main goal of each of your supports with Whaiora.
**He kōrero mō te tūranga | The Role**
You will provide community-based support advancing recovery and wellbeing goals for Whaiora in primary healthcare alongside other professionals while overcoming challenges that practice based care is unable to resolve.
This role requires strong communication skills with other members of the practitioner team and innovative solutions to problems with no simple answer. You will be well supported both internally and externally with a wide range of services and other initiatives you can work with to find the best support options for everyone.
This role is highly flexible both in the supports you provide and the people needing them. One moment you can be supporting someone with their health literacy, then someone with social services or maybe supporting someone with their mental health while navigating more long-term services to meet their needs.
We are looking for 1 x Permanent, part time staff member to work 16 hours between the hours of 8am - 4.30pm, Monday - Friday. So this role would suit an experience Support Worker looking for reduced hours.
**He kōrero mōu | About You**
We are looking for a culturally respectful and non-judgmental individual to provide support to Tangata Whaiora in the community from a wide range of cultures and identities, while working with professional health and wellbeing workers.
You'll need to be a problem solver, quicker thinker and able to connect Whaiora with a wide variety of services whilst also exploring new ones alongside them which includes the various day to day frustrations people have when they have reached their breaking point and need help.
Celebrating the successes of individuals and helping them plan their next steps as they move past needing your support, goes without saying. Being self-motivated and working well semi-autonomously with strong time management skills are a must.
To be successful in this role you will need experience in mental health and wellbeing support. A Level 4 in Health and Wellness would be desirable. Previous knowledge and experience in community support, or helping individuals navigate frustrating and complex wellbeing or social services would be beneficial.
Cultural and LGBTQ experience would be highly applicable. Above all else being positive, flexible, understanding, compassionate and driven to help people overcome the challenges in their lives is a must.
