The Older Persons Mental Health
Community Team has a part-time opportunity for an experienced Social Worker,
who has a passion for working in mental health and enthusiasm to make a
difference in the lives of older persons. This role involves working directly
with clients/tangata whai ora, whanau, NGO service providers, Primary Health
sector and Aged Residential Care facilities with the support of our wider
Multi-disciplinary team. The role also involves working alongside the team at
the referral centre as required, managing intake and acute situations.
This is an
exciting and rewarding opportunity to provide quality care to older people
within the Canterbury community.

**The Role**: Where

**In this role, you will be responsible for**:

- Working with individuals,

groups and their families/whanau experiencing mental health and/or dementia with a
focus on psychiatric assessment and care coordination;
- Completing mental health assessments;
- Developing client-focussed
and engaging care plans;
- Working with consumers from
diverse social and cultural backgrounds and working collaboratively with
whanau, the wider multidisciplinary team, and other relevant agencies to
ensure the best outcomes;
- Action interventions,
requests for information, and providing reports within adequate
timeframes;
- Identifying and contributing
towards service improvement initiatives;Your skills will include:

- A flexible and adaptable approach to your work;
- Good interpersonal skills, with ability to form relationships with clients and families under stress;
- Registration as a Social Worker with current Annual Practicing Certificate is essential;
- Strong clinical post-graduation experience;
- Experience within the mental health sector and/or older person's health is preferable;
- The ability to undertake InterRAI assessments is advantageous however training can be provided.Ngā hua |
