**About the Role**

In this role, you will manage a case load and will:

- Be involved in MDT assessments, treatment planning, reviews, and discharge planning.
- Contribute a social work perspective to the multi-disciplinary team with a focus on systemic and psychosocial factors impacting tāngata whaiora and their whānau
- Provide input into reports and complete psychosocial assessments for tāngata whaiora
- Provide whānau support and whānau liaison with the tāngata whaiora and the service
- Complete risk screening and safety planning relating to child protection and family violence
- Safeguard patient rights, through holding a broad knowledge of legislation
- Support equity in service provision for tāngata whaiora who are Maori and their whānau
- Hold a good working knowledge of support services and government agencies which support tāngata whaiora to live well in the community.
- Hold a good understanding of the interface between Specialist Mental Health Services, Department of Corrections and Ministry of Health
- Complete required note writing and data entry to support safe practice
- Sustain professional boundaries and attend supervision to maintain robust social work practice

**About the Team/Service/ Location**

Specialist Mental Health Services are an interdisciplinary team which forms part of Te Whatu Ora - Waitaha (Canterbury).

The Community Forensic Team, based at Hillmorton Hospital provides assessment, treatment, education, training, and community focused care for tāngata whaiora who have a mental disorder who have offended. We are a small, multidisciplinary team who are big on supporting each other, and we follow an assertive outreach model of care - you will get to know not only the tāngata whaiora in your own caseload, but also those under the team - s care.

**About you**

Your skills will include:

- Whakamanākitanga - Mana enhancing practice which respects and supports the tīno rangatiratanga of each individual
- Whakawhānaungatanga - Ability to build relationships and work collaboratively with tāngata whaiora, their whānau and the multi-disciplinary team
- Use of social work perspectives and models of practice which support your understanding of the holistic needs of tāngata whaiora and their whānau
- An understanding of Te Tiriti O Waitangi and how this relates to equitable practice
- Ability to prioritise and plan your workload-based on the service needs
- Technical literacy and expert data entry insuring safe practice

For this role you will need:

- Registration with the Social Work Registration Board and hold a current annual practice certificate.
- For those with limited experience in clinical mental health, additional training will be a contractual requirement of your employment and will be provided.
