The remuneration range for this role is $72,074 to $86,719 pro rata and is covered by the collective agreement PSA Allied MECA.
- Based in Te Ara Tiaki Pakeke, Healthy Ageing Service, at Wai Ora on the Gisborne Hospital campus.
- The hours of work or rostering pattern is negotiable between Monday - Friday

Position Description: CSB Co-ordinator
**About the Role**
We are seeking a person with a strong interest in working in community as a member of our inter-disciplinary service, to lead the coordination of the Community Strength and Balance group classes under the ACC Live Stronger for Longer programme. Te Ara Tiaki Pakeke is the lead agency for this programme, which is delivered in partnership with ACC, the Ministry of Health and the Heath Quality & Safety Commission NZ.
The key result areas are:
- Programme Coordination
- Undertake programme coordination of the existing network of Community Strength and Balance (CSB) classes,
- Establish additional approved (CSB) classes
- Increase the engagement with, and attendance at, evidence based strength and balance initiatives for older adults at risk of falling
- Improve equity of access to meet demand, cultural diversity and affordability.
- Reporting
- Build Community Group Strength & Balance momentum, including initiatives on the East Coast.
- Provide reports to the service manager outlining insights and progress of the work programme
**About the Team/Service/ Location**
Te Ara Tiaki Pakeke (TATP) is the healthy ageing service at Health NZ Te Whatu Ora Tairāwhiti, which assists older people to _live well, age well and have a respectful end of life in age-friendly communites_.
TATP is a community facing service that works with older persons (pakeke) and their whānau, across the health spectrum of care. TATP is not constrained by traditional models of primary, secondary and tertiary care, rather we work to enable pakeke, their whānau, together with their key providers, to maintain their independence in community, address developing needs, access health services and supports, and plan for their future.
We take a holistic approach that prioiritises listening to older persons, to supporting their life journey by coordinating and integrating healthcare needs.
