**Mō tēnei mahi (About this job)**
We are seeking a dedicated **Transition Coach - Youth Services** tojoin us on a **permanent, full-time (37.5 hours per week)** basis based in**Hamilton**
As a **Transition Coach,** you will work in partnership with Oranga Tamariki (OT) and other agencies to provide holistic and culturally responsive support to eligible rangatahi aged 15 years - 21 years. This role requires you to work within the standards of Oranga Tamariki and the values -driver service delivery of Te Wānanga Io Aotearoa.
Some of the responsibilities in this role will include:
- Respond to referrals of rangatahi to the service, by Oranga Tamariki Social Workers.
- Support rangatahi to actively participate in assessment and planning processes.
- Respond to referrals of rangatahi to the service by Oranga Tamariki (OT) social workers.
- Support rangatahi to actively participate in assessment and planning processes.
- Support rangatahi to access and engage with appropriate services and development opportunities to help them overcome barriers to reaching adult independence.
- Assist rangatahi to build, strengthen and maintain connections with whanau, hapū iwi and a broader network of supportive adults.
- Provide practical, emotional, mentor support and skill development to rangatahi as per transition plans.
- Meet the reporting requirements of Oranga Tamariki and TWoA.
**Mōu (About you)**
We are looking for a compassionate, culturally competent, and motivated individual who has a genuine passion for supporting rangatahi to achieve their goals and aspirations.
You will also bring with you:
- A Bachelor Degree in Social Work or Youth Work.
- Current professional body registration (SWRB NZ).
- 4+ years’ experience in social work, youth services or a similar type of role.
- 4+ years’ experience working with Māori or Pasifika.
- Proven experience providing transitional and social services to rangatahi.
- A full NZ Drivers Licence This position is classed as a Core Worker and will need to pass a safety check, inclusive of a Police Vetting under the Children’s Act 2014 and a Cyrus Check (OT).
As a kaupapa Māori organisation, your willingness to participate in cultural activities and develop an understanding of Āhuatanga Māori (values, culture and tikanga) will be key.
